The yearlong collaboration comprised a wide variety of ‘experts’ including clinicians, patients, service providers, health care providers and medical teams to co-design a pathway focussed on local access and continuous care and connection for women and their families.
The project was led by Albury Wodonga Health’s Jan Lang and included focus on collaborative relationships between health professionals to create a holistic service for women in the community that is uniquely tailored to the individual and encompasses care before, during and after birth.
This includes, antenatal care, delivery and post-natal care from multidisciplinary teams including obstetricians, GP obstetricians, midwives and allied health.
The innovative partnership provides a seamless pathway for women to be able to choose to access local contemporary care close to home, supported by a unique and personal birthing experience close by at Northeast Health Wangaratta with the additional option to return to their local community via Yarrawonga Health for post-natal follow up and support.
The partnership provided the opportunity to explore new models of contemporary and effective care, with clearly identified consultation and referral pathways supported by both Yarrawonga Health and Northeast Health Wangaratta teams.
“Yarrawonga Health is proud to have embarked on such an innovative project partnership that undoubtedly provides women in Yarrawonga and surrounds the opportunity to continue to access local, contemporary and individualised maternity care close to home, backed by a regionally based birth,” Midwife Amy Jury said.
“Northeast Health Wangaratta has been instrumental in us achieving a woman first pathway co-designed to deliver exceptional maternity service for local women and their families.
“We are excited to be able to support mothers through this seamless pathway andprovide quality local care.”
