Ryan Cho, 28, faced Melbourne Magistrates Court on Friday, charged with 910 offences including stalking and producing an intimate image.
It's alleged he concealed a phone in a mesh bag inside staff toilets at the Austin Hospital, the Peter MacCallum Cancer Centre and the Royal Melbourne Hospital.
Material found on the phone allegedly shows staff using the toilet or shower facilities, with their genital and anal areas exposed.
Police allege the files were created between 2021 and 2025, and were categorised into different folders referencing each of the three hospitals.
Cho was initially charged and remanded in custody in July on five charges but he successfully applied for bail in the Supreme Court in August.
In the months that have followed, police have laid hundreds of additional charges, with the total now at 910.
During his brief hearing on Friday, magistrate Michelle Mykytowycz said Cho had notified the court on February 16 that he had changed solicitors.
Defence lawyer Kristina Kothrakis confirmed her firm was now acting for the 28-year-old and they would need time to go through the extensive brief of evidence.
She sought a longer than usual adjournment, which was not opposed by prosecutors.
Ms Mykytowycz granted the adjournment to May, noting it was appropriate but flagging the case should progress at the next hearing.
The magistrate also acknowledged some of Cho's alleged victims and their supporters who were seated in the courtroom or watching over the video link.
Cho's bail was extended to his next committal mention date.
He tried to avoid reporters waiting outside by remaining in the court building for more than two hours, before emerging with his face hidden behind a mask, sunglasses, cap and pink folder.
